Healthcare Directives Information for Hospice Patients


When dealing with a life-limiting illness, making plans and decisions in advance is not only wise, it helps avoid an enormous level of stress and concern now and in the future. When a crisis does arise, the necessary paperwork will be prepared and everyone involved will already be in agreement, allowing patients and their loved ones to make the most of the time at hand.

Faith Presbyterian Hospice respects and supports patients’ rights to make their own medical treatment decisions. Texas State law authorizes any legally competent adult (18 years of age or older) to make his or her own decisions concerning medical and surgical treatment. These decisions can range from routine medical treatment to withholding of nutrition and hydration (food and water).

Our policy is to inquire about the existence of previously executed Advance Directives and to offer all patients the opportunity to sign a Directive to Physicians and Family or Surrogates, and/or an Out of Hospital Do Not Resuscitate directive. Whether or not a patient chooses to utilize an Advance Directive is a personal matter and will never be a condition of providing care or basis for discrimination for or against a patient.
